What it is

HCG is a licensed medicine (e.g. Pregnyl), primarily approved for fertility indications. In TRT contexts, it's used off-label because it mimics LH, signalling the testes to keep producing testosterone and sperm even while external testosterone is being administered.

This off-label TRT-adjunct use is common in specialist hormone clinics and is generally considered good practice by many endocrinologists for men wanting to preserve fertility, even though it isn't HCG's originally licensed indication.

What’s claimed — and how strong the evidence behind each claim actually is.

“Preserves fertility and testicular size during TRT”

Mechanistically sound (mimics LH) and widely used in specialist practice, though large dedicated RCTs for this specific off-label use are limited.

“A substitute for stopping TRT rather than an adjunct to it”

Its role is to support the testes alongside TRT, not to replace or replicate what TRT does.
